Emmanuel is a 60 year old male that was admitted to Chatham Hills Subacute Care Center after his recent hospitalization at Overlook Hospital. Prior to his hospitalization, he was completely independent and employed full-time. His COPD was fully treated upon hospitalization and shortly after he was admitted to Chatham Hills Subacute for rehabilitation with the intent of achieving the following short and long term goals:
- Patient to increase strength to facilitate improved functional mobility
- Patient to safely perform functional transfers with correct hand/food placement for initiation/termination of tasks
- Implementation of safety techniques with ability to achieve/maintain balance to facilitate increased participation with daily activities
- Patient to safely ambulate on level surfaces using platform walker
- Patient to safely perform toileting tasks using raised toilet seat/commode
- Patient will safely and efficiently perform adaptive equipment
- Patient to increase standing balance during activities of daily living (ADLs)
- Patient to complete toilet/commode transfers
- Patient to learn how to administer breathing treatments safely
After completing both physical and occupational therapy to attain all of the above mentioned goals, the patient made consistent progress. His functional abilities progressed as a result of skilled interventions, and the patient reached maximum potential with skilled services. Upon initial evaluation, he required maximum assistance for transfers and had increased difficulty walking and wheelchair mobility, requiring increased assistance. Patient was discharged having attained all of his goals with safe wheelchair mobility and walking with a platform walker!
